Location & Times

Passport Applications are accepted by appointment only at the Umatilla. County Courthouse located at 216 SE 4th St., Pendleton, Oregon 97801.  Schedule your appointment using the appointment calendar at the bottom of this page.  Please allow 15 to 30 minutes per applicant.

Appointments are available from 12:00 p.m. through 4:00 p.m. on Mondays and from 8:00 a.m. to 4:00 p.m. Tuesday through Friday.  All forms & documents are available on the U.S. Department of State website (http://travel.state.gov).

If you are an adult needing to renew your passport, please visit the State Dept. website first.  You may qualify to renew on your own through the mail and will save $35-$50.

INSTRUCTIONS FOR FILLING OUT PASSPORT APPLICATION

Type or Print using BLACK INK ONLY to fill out the application form through Box 21.

DO NOT SIGN APPLICATION - EACH APPLICANT MUST APPEAR IN PERSON

Bring the completed application and items listed below with you to your scheduled appointment at the County Records Office in Room 116:

A DOCUMENT ESTABLISHING CITIZENSHIP:

Government issued certified copy of your birth certificate

Certificate of Naturalization

Certificate of Citizenship

Consular Report of a Birth Abroad

Previously issued US Passport

(Hospital issued birth certificates, US Census Certificates, US Birth Registrations and most Birth Record Abstracts are not acceptable) 

(To establish parental or custodial relationship, an applicant 15 years of age or younger renewing their passport, must also submit a State Certified Copy of their Birth Certificate together with the previously issued US Passport.) 

ONE FACE-FRONT PHOTOGRAPH (2" x 2", Color only with a white background), in which eyes are clearly shown. (No glasses whatsoever, bows, hats or any type of uniform). The County Records Office takes passport photos for $15.00

VALID PHOTO ID Present a valid Driver’s License or other valid Government issued photo ID with your signature on it. (If you ID is not from Oregon, you will need a secondary piece of ID)

Children 15 years of age and younger need both parents or legal guardian(s) to appear and sign the application. A Single Parent/Legal Guardian(s) with full Custody needs Court Certified Documentation to submit with each child’s application. Children 16 and 17 need either parent or legal guardian with valid photo ID to appear and sign the application.

ONE CHECK OR MONEY ORDER PER APPLICANT TO PAY THE FEDERAL FEE. (cash & debit/credit cards not accepted )

YOU MAY APPLY FOR A US PASSPORT BOOK, A US PASSPORT CARD, OR BOTH PLEASE NOTE: The US Passport Card may only be used for international travel by land or sea between the United States, Canada, Mexico, the Caribbean and Bermuda.  ALSO NOTE THAT THERE ARE 2 SEPARATE FEES.

 

Cards & Cash are not accepted for the Federal Fee.  Personal Check, Cashier’s Check, or Money Order only, made out to: "US Dept. of State"
